摘要

目的 了解2022—2024年阜阳市颍泉区中小学生筛查性近视情况及其影响因素,为中小学生近视防控工作提供科学依据。方法 采用多阶段整群随机抽样的方法抽取颍泉区2022—2024年中小学生7 096人,对其开展远视力、屈光检查,并对5 259名四年级及以上的学生开展问卷调查,计算筛查性近视检出率,采用二元logistic回归进行多因素分析。结果 2022—2024年阜阳市颍泉区中小学生筛查性近视率分别为60.23%、57.27%、56.50%。从不同学段来看,颍泉区小学、初中、高中学段学生筛查性近视率分别为37.50%、67.79%、88.40%,整体呈现上升趋势(χ ${\mathrm{ }}_{\mathrm{趋}\mathrm{势}}^{2}$=1 330.387,P<0.001); 从性别来看,男、女生筛查性近视率分别为54.13%、62.45%,女生近视检出率高于男生,差异有统计学意义(χ2=50.164,P<0.001)。二元logistic回归分析结果显示,每天户外活动时间长≥2 h(OR=0.800,95%CI:0.695~0.920)、眼睛距离书本1尺(OR=0.863,95%CI:0.756~0.986)是中小学生近视发生的保护因素;女性(OR=1.402,95%CI:1.234~1.593)、初中(OR=2.055,95%CI:1.740~2.427)和高中学段(OR=7.392,95%CI:5.831~9.372)、仅父亲近视(OR=1.825,95%CI:1.459~2.283)或仅母亲近视(OR=2.031,95%CI:1.642~2.513)和父母均近视(OR=2.372,95%CI:1.785~3.152)是中小学生发生近视的危险因素。结论 颍泉区中小学生筛查性近视检出率较高,筛查性近视受多种因素的影响,应采取有效措施提升防控效果、保护学生视力。

Abstract

Objective To understand the prevalence and influencing factors of screening myopia among primary and secondary school students in Yingquan District of Fuyang from 2022 to 2024,and to provide scientific basis for myopia prevention and control among primary and secondary school students. Methods A multi-stage cluster random sampling method was used to select 7 096 primary and secondary school students in Yingquan District from 2022 to 2024.Their distance vision acuity and refractive status were examined,and a questionnaire survey was conducted among 5 259 students in grade four and above.The detection rate of screening myopia was calculated,and binary logistic regression was used for multivariate analysis. Results From 2022 to 2024,the detection rates of screening myopia among primary and secondary school students in Yingquan District of Fuyang were 60.23%,57.27%,and 56.50%,respectively.From the perspective of different educational stages,the detection rates of screening myopia in primary school,junior high school,and senior high school students in Yingquan District were 37.50%,67.79%,and 88.40%,respectively,showed an overall upward trend (χ ${\mathrm{ }}_{\mathrm{t}\mathrm{r}\mathrm{e}\mathrm{n}\mathrm{d}}^{2}$=1 330.387,P<0.001).In terms of gender,the detection rates of screening myopia in male and female students were 54.13% and 62.45%,respectively,with statistically significant difference (χ2=50.164,P<0.001).The results of binary logistic regression analysis showed that daily outdoor activity time≥2 hours (OR=0.800,95%CI:0.695-0.920),and keeping eyes one foot away from books (OR=0.863,95%CI:0.756-0.986) were protective factors against myopia in primary and secondary school students.Female (OR=1.402,95%CI:1.234-1.593),studying in junior high school (OR=2.055,95%CI:1.740-2.427) or senior high school (OR=7.392,95%CI:5.831-9.372),paternal myopia only (OR=1.825,95%CI:1.459-2.283) or maternal myopia only (OR=2.031,95%CI:1.642-2.513),and both parents with myopia (OR=2.372,95%CI:1.785-3.152) were risk factors for myopia in primary and secondary school students. Conclusion The detection rate of screening myopia among primary and secondary school students in Yingquan District is relatively high,and screening myopia is affected by various factors.Effective measures should be taken to improve the effectiveness of prevention and control,and protect students' vision.

The prevalence of myopia in children is increasing worldwide and is viewed as a major public health concern. This increase has driven interest in research into myopia prevention and control in children. Although there is still uncertainty in the risk factors underlying differences in myopia prevalence between ethnic groups, rates in children of East Asian descent are typically higher regardless of where they live. Mounting evidence also suggests that myopia prevalence in children increases with age. Earlier commencement and more rigorous education systems in these countries, resulting in more time spent on near-work activities and less time on outdoor activities, may be responsible for the earlier age of myopia onset. However, to date, the mechanisms regulating myopia onset and progression are still poorly understood. Findings from several studies have shown orthokeratology to be effective in slowing axial elongation and it is a well-accepted treatment, particularly in East Asian regions. While our understanding of this treatment has increased in the last decade, more work is required to answer questions, including: How long should the treatment be continued? Is there a rebound effect? Should the amount of myopia control be increased? To whom and when should the treatment be offered? Practitioners are now faced with the need to carefully guide and advise parents on whether and when to undertake a long somewhat complex intervention, which is costly, both in time and money. In the near future, a greater demand for effective prophylaxis against childhood myopia is envisaged. Other than orthokeratology, atropine therapy has been shown to be effective in slowing myopia progression. While its mechanism of control is also not fully understood, it is likely that it acts via a different mechanism from orthokeratology. Thus, a combined treatment of orthokeratology and atropine may have great potential to maximise the effectiveness of myopia control interventions.© 2018 Optometry Australia.

There is an epidemic of myopia in East and Southeast Asia, with the prevalence of myopia in young adults around 80-90%, and an accompanying high prevalence of high myopia in young adults (10-20%). This may foreshadow an increase in low vision and blindness due to pathological myopia. These two epidemics are linked, since the increasingly early onset of myopia, combined with high progression rates, naturally generates an epidemic of high myopia, with high prevalences of "acquired" high myopia appearing around the age of 11-13. The major risk factors identified are intensive education, and limited time outdoors. The localization of the epidemic appears to be due to the high educational pressures and limited time outdoors in the region, rather than to genetically elevated sensitivity to these factors. Causality has been demonstrated in the case of time outdoors through randomized clinical trials in which increased time outdoors in schools has prevented the onset of myopia. In the case of educational pressures, evidence of causality comes from the high prevalence of myopia and high myopia in Jewish boys attending Orthodox schools in Israel compared to their sisters attending religious schools, and boys and girls attending secular schools. Combining increased time outdoors in schools, to slow the onset of myopia, with clinical methods for slowing myopic progression, should lead to the control of this epidemic, which would otherwise pose a major health challenge. Reforms to the organization of school systems to reduce intense early competition for accelerated learning pathways may also be important.Copyright © 2017 Elsevier Ltd. All rights reserved.
