目的 分析2013~2015年合肥地区临床患者中分离的沙门氏菌的血清分布及耐药特征,为沙门氏菌感染的患者提供临床治疗依据。 方法 根据食源性疾病监测方案,对从临床患者粪便中分离的沙门氏菌进行分离鉴定,阳性菌株用全自动细菌鉴定仪做生化鉴定和药敏分析。 结果 2013~2015年共分离75株沙门氏菌,其中肠炎和鼠伤寒两种血清型占57.0%(43/75),耐药率达85.3%(64/75),耐多药率达到24.0%(18/75);肠炎血清型有95.0%(19/20)的菌株耐萘啶酸,而鼠伤寒血清型耐四环素的菌株达到74.0%(17/23);从儿童粪便中分离菌耐多药的比例为22.7%(15/66),成人分离菌无耐多药现象。 结论 沙门氏菌在合肥地区的血清型以肠炎和鼠伤寒为主,呈现多样化的特点;耐药情况较严重,但对临床常用的抗菌药敏感性较好;儿童分离菌耐多药现象应注意防范。
Abstract
Objective To analyze the serotype distribution and antimicrobial resistance of salmonella strains isolated from clinic patients in Hefei from 2013 to 2015, and provide references for the prevention and clinical treatment of salmonella infection. Methods All strains which were isolated from feces of clinic patients were separated and identified according to the guide of food-borne disease surveillance, and then positive strains were conducted for biochemical tests and antibiotic resistance analysis by automated microbiology analyzer. Results 75 salmonella strains were isolated in 2013~2015. S.Typhimurium and S.Enteritidis count 57.0%(43/75) of all the positive strains. The drug resistance rate was 85.3%(64/75), while multi-drug resistance rate was 24.0%(18/75). S. Enteritidis showed as high as 95.0%(19/20) resistance to nalidixic acid while S.Typhimurium showed 74.0% (17/23)resistance to tetracycline. 22.7%(15/66)strains isolate from children were multi-drug resistance, while no multi-drug resistant strain showed in adult. Conclusion S.Typhimurium and S.Enteritidis were the most frequently isolated salmonella serovars from clinic patients in Hefei, and meanwhile there were other diversity serovars.Though the drug resistance was severe, strains hadn't showed high resistance to clinical commonly used drugs. Precautions should be paid on multi-drug resistance phenomenon of the strains from children.
