摘要

目的 了解8~10岁儿童不同时段尿碘水平,探讨随意尿样代替晨尿的可行性。方法 采取整群分层抽样方法,分别抽取合肥市城区和农村各1所小学,每所小学随机抽取8、9和10周岁儿童各20名(男、女各半),共计120人,资料齐全纳入分析的儿童118人。采集每名儿童当天10∶00、12∶30、21∶30中段尿和次日晨尿,对采集的尿样进行尿碘含量测定。结果 118名儿童四个不同时段尿样尿碘含量分别为174.4 μg/l、232.1 μg/l、259.7 μg/l和195.0μg/l,差异有统计学意义(M=17.254,P<0.001);按照小于100μg/l、100~299 μg/l和300 μg/l及以上尿碘样品分布,差异也有统计学意义(M=12.659,P=0.005)。12∶30尿样与与21∶30尿样、次日晨尿尿碘含量及尿碘频数分布差异均无统计学意义(P>0.05)。晨尿尿碘与其他三个时段尿碘水平均有一定相关关系,其中12∶30中段尿与次日晨尿尿碘水平相关性最高,相关系数为0.593。轮廓分析显示儿童当天10∶00尿碘水平最低,然后逐渐上升,21∶30达到最高,次日晨起尿碘有所下降。结论 12∶30尿样代替晨尿进行尿碘水平监测可行。

Abstract

Objective To understand the urine iodine level of 8-10 year old children at different periods of the day,and explore the feasibility of random urine instead of morning urine. Methods According to cluster stratified sampling method, we chose one urban and one rural primary schools in Hefei. From children aged 8, 9 and 10, We selected 20 people of each age respectively in each school with stratified random sample method, half male and half female. There were totally 120 children,There were 118 children with information completely to be analyzed.We collected the urine samples at 10∶00 am, 12∶30 pm, 21∶30 pm and in the next morning, and detected urine iodine. Results The urinary iodine contents of 118 children in four different periods were 174.4ug/l, 232.1ug/l, 259.7ug/l and 195.0ug/l, respectively. The difference was statistically significant(M=17.254,P<0.001). The difference was also statistically significant in urine iodine samples of less than 100 mu g/l, 100 ~299 g/l and 300 g/l and above(M=12.659,P=0.005). There was no significant difference between 12:30 urine sample and 21:30 urine sample, the next morning urine iodine content and urine iodine frequency distribution(P>0.05). Urinary iodine in the morning was correlated with urinary iodine levels in other three periods. The correlation coefficient between urinary iodine levels in the middle of 12:30 and the next morning was the highest, and the correlation coefficient was 0.593. The profile analysis showed that the urinary iodine level of children was the lowest at 10:00 am on the same day, then gradually increased, reached the highest at 21:30 am and decreased the next morning. Conclusion It is feasible to monitor urine iodine level with 12:30 urine instead of morning urine.
