摘要

目的 回顾分析合肥市某高校一起非O1/非O139群霍乱弧菌引起的食源性疾病调查处置过程,探讨事件发生原因,为类似事件处置提供参考。 方法 对患者和食堂从业人员进行流行病学调查并采集肛拭子和病人呕吐物;对食堂及食品加工过程、环境卫生及水质状况进行现场调查和采集相关样本,采集的样品进行微生物检测。采用病例对照方法进行危险因素分析。 结果 本次事件共发病91人,96.7%患者以腹泻为主,潜伏期在7~48 h,在2份留样食品、11名患者、2名食堂A员工粪便/肛拭和1份餐具等涂抹样中检出非O1/非O139群霍乱弧菌。病例对照研究显示,食堂A是危险因素,OR95%CI为2.77~5.33,且发病与到食堂A就餐次数呈剂量反应关系(χ2=19.44,P<0.001)。多因素logistic回归分析,5月29日中餐、30日早餐和中餐是可疑餐次。 结论 该起事件由非O1/非O139群霍乱弧菌引起的食源性疾病暴发,原因可能为学校食堂员工污染食物和餐具引起。

Abstract

Objective To investigate an outbreak caused by non-O1 and non-O139 serogroups cholera in an university in Hefei and analyze the causes of the incident, so as to provide references for prevention and control strategies of similar cases in the future. Methods Epidemiological investigations were conducted on patients and staff working in the canteen; and anal swabs and patient vomit were collected.Canteen, food processing, environmental sanitation and water quality were also investigated.Related specimens were collected and detected.Case-control study was used analyze risk factors. Results There were 91 cases in the event, and 96.7% of patients had diarrhea, which was the main symptom.The incubation was among 7-48 hours.The non-O1 and non-O139 serogroups cholera was detected from 2 employees, 11 sufferers, 2 reserved samples and the specimens of tablewares.The case-control studies showed that the canteen A was the risk factor,OR95%CI= 2.77-5.33,and outbreak and times of taking meals had a dose response relationship(χ2=19.44, P<0.001).By the multivariate analysis, the lunch on May 29th, the breakfast and lunch on May 30th were independent variables. Conclusion The foodborne outbreak was caused by non-O1 and non-O139 serogroups cholera, which might be caused by the food and tableware contaminated by the canteen workers.
