目的 了解芜湖市基层医疗卫生机构公共卫生服务资源配置与传染病预防控制服务现状及其人口和地理分布均等化程度。方法 通过现况调查了解芜湖市基层医疗卫生机构公共卫生资源配置和预防接种、传染病报告、卫生监督协管等传染病预防控制服务现状,按服务覆盖人口和地理面积,应用基尼系数评估其均等化程度。结果 调查305家基层医疗卫生机构共有在职人员3 690人,预防保健专职人员737人,服务覆盖人口312.4万人、覆盖地理面积3 606.2 km²。机构房屋面积平均为724.6 m²,常用设备种类齐全率为65.5%,调查6个剂次疫苗仅3个剂次接种率达90%以上。被调查机构2012年传染病报告率为98.1%,及时报告率为99.5%;卫生监督协管信息报告率为97.6%。机构在职人员、预防保健专职人员、房屋面积、常用设备种类、预防接种、传染病报告、卫生监督协管按人口累计的基尼系数分别为0.06、0.04、0.07、0.03、0.07、0.02、0.11;按地理面积累计的基尼系数分别为0.25、0.34、0.25、0.28、0.26、0.33、0.17。结论 芜湖市基层医疗卫生机构公共卫生服务资源配置和预防接种、传染病报告、卫生监督协管按人口与地理面积分布均等化程度相对较高,但预防接种率仍有待提高。

Objective To understand the status of public health service resources allocation and prevention and control of infectious diseases in primary medical and health institutions in Wuhu, and to determine the degree of population and geographical distribution. Methods A survey was conducted to learn the current status of the primary health institutions of public health services resource, vaccinations, infectious disease reporting, health supervision assist and so on in Wuhu. The equalization was calculated by Gini coefficient according to the population and geography information. Results 305 institutions had a total of 3,690 staff members, and 737 full-time public health service staff members, covering a population of 3,124,000 people and the geographical area of 3606.2 km2. The average area of agencies building was 724.6m2. The rate of common equipment types was 65.5%, the vaccine of vaccination rate up to 90% was only three doses in the six doses survey vaccine. The rate of infectious disease reporting was 98.1% and the rate of timely reporting was 99.5%. The information reporting rate of health supervision assist was 97.6% in 2012. The demographic Gini coefficients of staff, full-time public health service members, building area, equipment types, vaccination, infectious disease reporting and health supervision assist of the primary health institutions were respectively 0.06, 0.04, 0.07, 0.03, 0.07, 0.02, 0.11, and the geographic Gini coefficients were respectively 0.25, 0.34, 0.25, 0.28, 0.26, 0.33, 0.17 respectively. Conclusion The demographic and geographic equalization level of public health services resource allocation, vaccinations, infectious disease reporting, health supervision assist were higher, but the vaccination rate still needs to be improved.
