Epidemiological characteristics of severe fever with thrombocytopenia syndrome in Linyi of Shandong Province from 2018 to 2023

Epidemiological characteristics of severe fever with thrombocytopenia syndrome in Linyi of Shandong Province from 2018 to 2023
Objective To understand the epidemiological characteristics of severe fever with thrombocytopenia syndrome (SFTS) in Linyi of Shandong Province from 2018 to 2023,and to provide basis for the prevention and control of SFTS. Methods The data of reported cases with SFTS in Linyi from 2018 to 2023 were collected.The temporal,geographical and demographic distribution characteristics of SFTS were described.Statistical analysis was performed by using SPSS 22.0 software. Results A total of 423 cases of SFTS were reported in Linyi from 2018 to 2023,with an incidence rate of 0.64/100 000,19 cases died,with a mortality rate of 4.49%.The peak incidence period was from May to October,and the reported cases accounted for 96.22% (407/423).The regional distribution was mainly in Mengyin County,accounting for 56.97% (241/423),followed by Yishui County and Fei County,accounting for 11.11% (47/423) and 10.87% (46/423),respectively.The age distribution was mainly between 50 and 79 years old,accounting for 86.05% (364/423).The occupational distribution was mainly dominated by farmers,accounting for 95.74% (405/423).There were statistically significant differences between death and non-death cases in terms of occupation (χ2=6.496,P=0.011),first diagnosis medical institution (χ2=7.251,P=0.007),comorbid chronic underlying diseases (χ2=12.623,P<0.001),and the interval from onset to first visit (Z=3.036,P=0.001). Conclusion The number of reported cases of SFTS in Linyi is increasing,and the regions reporting SFTS are expanding.The incidence of SFTS mainly occurs from May to October,and the affected population is primarily elderly individuals in rural areas.

From June 2011 to August 2014, 21 cases of infection by severe fever with thrombocytopenia syndrome bunyavirus (SFTSV) were confirmed in Zhoushan Islands in the Eastern coast of China. To identify the source of SFTSV in Zhoushan Islands, the whole SFTSV genomes were amplified and sequenced from 17 of 21 patients. The L, M and S genomic segments of these SFTSV strains were phylogenetically analyzed together with those of 188 SFTSV strains available from GenBank. Phylogenetic analysis demonstrated SFTSV could be classified into six genotypes. The genotypes F, A and D were dominant in mainland China. Additionally, seven types of SFTSV genetic reassortants (abbreviated as AFA, CCD, DDF, DFD, DFF, FAF and FFA for the L, M and S segments) were identified from 10 strains in mainland China. Genotype B was dominant in Zhoushan Islands, Japan and South Korea, but not found in mainland China. Phylogeographic analysis also revealed South Korea possible be the origin area for genotype B and transmitted into Japan and Zhoushan islands in the later part of 20th century. Therefore, we propose that genotype B isolates were probable transmitted from South Korea to Japan and Zhoushan Islands.
