Epidemic characteristics and trend of infectious diseases among kindergarten children in Anhui Province from 2014 to 2023

Abstract

Objective To analyze the epidemic characteristics and trend of infectious diseases among kindergarten children in Anhui Province,and to provide reference for the development of prevention and control strategies for infectious diseases in children. Methods Descriptive epidemiological method was used to analyze the surveillance data of infectious diseases among kindergarten children in Anhui Province from 2014 to 2023.Joinpoint regression model was used to analyze the trend of incidence rate in different years. Results A total of 372 745 cases of 32 infectious diseases were reported in Anhui Province from 2014 to 2023 among children in childcare institutions,with an average annual incidence rate of 1 876.60/100 000,showed a stable trend (AAPC=12.79%,P=0.064).The top five cumulative reported cases were hand-foot-mouth disease (48.49%),influenza (27.74%),chicken pox (9.78%),other infectious diarrheal diseases (7.32%),and mumps (4.58%).The reported incidence rate of hand-foot-mouth disease decreased slowly (AAPC=-4.04%,P=0.308),mumps showed an upward trend (AAPC=1.67%,P=0.793),but had no statistically significant difference.The reported incidence rate of influenza (AAPC=66.54%,P<0.001) and other infectious diarrheal diseases (AAPC=15.43%,P<0.001) showed an upward trend.The reported incidence rate of chickenpox showed a rising-falling trend,with an upward trend from 2014 to 2019 (APC=32.11%,P<0.001),and a decreasing trend from 2019 to 2023 (APC=-12.89%,P=0.008).The incidence of infectious diseases in kindergarten children showed a bimodal type,the peak incidence of infectious diseases was from April to June and from October to January of the following year.The proportion of intestinal infectious diseases was high in boys (58.46%) and children under 3 years old (82.91%). Conclusion The incidence of infectious diseases among kindergarten children in Anhui Province is relatively high,and the incidence trend is stable.Intestinal and respiratory infectious diseases account for the main part,and the seasonal characteristics are obvious,so it is necessary to take targeted prevention and control measures according to the epidemic characteristics and the actual situation,thus to effectively control the occurrence and epidemics of common infectious diseases in nursery children.

The public health interventions to mitigate COVID-19 could also potentially reduce the global activity of influenza. However, this strategy's impact on other common infectious diseases is unknown. We collected the data of ten respiratory infectious (RI) diseases, influenza-like illnesses (ILI), and seven gastrointestinal infectious (GI) diseases during 2015-2020 in China and applied two proportional tests to check the differences in the yearly incidence and mortality, and case fatality rates (CFRs) over the years 2015-2020. The results showed that the overall RI activity decreased by 7.47%, from 181.64 in 2015-2019 to 168.08 per 100,000 in 2020 (p < 0.001); however, the incidence of influenza was seen to have a 16.08% escalation (p < 0.001). On the contrast, average weekly ILI percentage and positive influenza virus rate decreased by 6.25% and 61.94% respectively in 2020 compared to the previous five years (all p < 0.001). The overall incidence of GI decreased by 45.28%, from 253.73 in 2015-2019 to 138.84 in 2020 per 100,000 (p < 0.001), and with the greatest decline seen in hand, foot, and mouth disease (HFMD) (64.66%; p < 0.001). The mortality and CFRs from RI increased by 128.49% and 146.95%, respectively, in 2020, compared to 2015-2019 (p < 0.001). However, the mortality rates and CFRs of seven GI decreased by 70.56% and 46.12%, respectively (p < 0.001). In conclusion, China's COVID-19 elimination/containment strategy is very effective in reducing the incidence rates of RI and GI, and ILI activity, as well as the mortality and CFRs of GI diseases. Infectious respiratory diseases are highly contagious and very common, and thus can be considered as one of the leading causes of morbidity and mortality worldwide. We followed up the incidence rates (IRs) of eight infectious respiratory diseases, including chickenpox, measles, pertussis, mumps, invasive pneumococcal disease, scarlet fever, rubella, and meningococcal disease, after COVID-19 mitigation measures were implemented in South Korea, and then compared those with the IRs in the corresponding periods in the previous 3 years. Overall, the IRs of these diseases before and after age- or sex-standardization significantly decreased in the intervention period compared with the pre-intervention periods (p < 0.05 for all eight diseases). However, the difference in the IRs of all eight diseases between the IRs before and after age-standardization was significant (p < 0.05 for all periods), while it was not significant with regard to sex-standardization. The incidence rate ratios for eight diseases in the pre-intervention period compared with the intervention period ranged from 3.1 to 4.1. These results showed the positive effects of the mitigation measures on preventing the development of respiratory infectious diseases, regardless of age or sex, but we need to consider the age-structure of the population to calculate the effect size. In the future, some of these measures could be applied nationwide to prevent the occurrence or to reduce the transmission during outbreaks of these infections. This study provides evidence for strengthening the infectious disease management policies in South Korea.

We aimed to determine the hospital burden of varicella-zoster virus infection (VZV) in England during 2004-2013 to support a future cost-effectiveness analysis of a childhood varicella vaccination programme.We analysed the incidence, duration, outcome and costs of hospitalisations for VZV using the Hospital Episode Statistics (HES) database for the general and immunocompetent population. Mortality in HES was validated using data from the Office for National Statistics (ONS).The average annual incidences of admissions due to varicella and herpes zoster were 7.6 (7.3-7.9) and 8.8 (8.6-9.0) per 100,000, respectively. The immunocompetent population accounted for 93% and 82% of the admissions due to varicella and herpes zoster, respectively. The average yearly number of hospital days was 10,748 (10,227-11,234) for varicella and 41,780 (40,257-43,287) for herpes zoster. The average yearly hospital costs (£2013/14) were £6.8 million (6.4-7.2) for varicella and £13.0 million (12.8-13.4) for herpes zoster. The average annual numbers of deaths identified in HES due to varicella and herpes zoster were 18.5 (14.3-22.8) and 160 (147-172), respectively. Comparison with ONS mortality data indicated a high level of uncertainty.Most of the hospital burden due to VZV-virus in England occurs in the immunocompetent population and is potentially vaccine-preventable.Crown Copyright © 2016.
