Analysis of influencing factors of the injury severity due to falls in Fuyang District of Hangzhou from 2021 to 2023

Analysis of influencing factors of the injury severity due to falls in Fuyang District of Hangzhou from 2021 to 2023
Objective To analyze the distribution characteristics and influencing factors of injury severity due to falls in Fuyang District of Hangzhou,and to provide scientific evidence for formulating prevention and control intervention policies. Methods The clinical data from hospital injury monitoring in Fuyang District from 2021 to 2023 were collected,and the epidemic characteristics were analyzed.Multivariate logistic regression was used to analyze the influencing factors of injury severity. Results A total of 16 261 cases of falls were collected.The severity of injury was mainly mild (10 133 cases),accounting for 62.31%.There were 6 128 moderate and severe cases,accounting for 37.69%.The multivariate analysis showed that 60-74 years old (OR=1.466,95%CI: 1.038-2.071),75 years old and above (OR=2.052,95%CI: 1.443-2.918),occupation of business service (OR=1.638,95%CI: 1.099-2.442),agricultural and fishery workers (OR=2.309,95%CI: 1.577-3.382),retired and unemployed persons (OR=2.137,95%CI: 1.246-3.667),workers (OR=2.558,95%CI: 1.747-3.747),injury occurred at home (including yard,etc.) (OR=1.638,95%CI: 1.142-1.901),activity when falls occurred was working (OR=2.373,95%CI: 2.005-2.808),housework (OR=1.135,95%CI: 1.027-1.255),school activities (OR=2.054,95%CI: 1.103-3.825),physical activities (OR=1.636,95%CI: 1.044-2.563) were risk factors of injury severity for falls. Conclusion Patients with moderate and severe injuries account for a large proportion of injury cases,and the influencing factors are various.Targeted prevention and control work should be carried out according to relevant risk factors.

This review updates evidence since the 2002 U.S. Preventive Services Task Force recommendation on osteoporosis screening.To determine the effectiveness and harms of osteoporosis screening in reducing fractures for men and postmenopausal women without known previous fractures; the performance of risk-assessment instruments and bone measurement tests in identifying persons with osteoporosis; optimal screening intervals; and the efficacy and harms of medications to reduce primary fractures.Cochrane Central Register of Controlled Trials and Cochrane Database of Systematic Reviews (through the fourth quarter of 2009), MEDLINE (January 2001 to December 2009), reference lists, and Web of Science.Randomized, controlled trials of screening or medications with fracture outcomes published in English; performance studies of validated risk-assessment instruments; and systematic reviews and population-based studies of bone measurement tests or medication harms.Data on patient populations, study design, analysis, follow-up, and results were abstracted, and study quality was rated by using established criteria.Risk-assessment instruments are modest predictors of low bone density (area under the curve, 0.13 to 0.87; 14 instruments) and fractures (area under the curve, 0.48 to 0.89; 11 instruments); simple and complex instruments perform similarly. Dual-energy x-ray absorptiometry predicts fractures similarly for men and women; calcaneal quantitative ultrasonography also predicts fractures, but correlation with dual-energy x-ray absorptiometry is low. For postmenopausal women, bisphosphonates, parathyroid hormone, raloxifene, and estrogen reduce primary vertebral fractures. Trials are lacking for men. Bisphosphonates are not consistently associated with serious adverse events; raloxifene and estrogen increase thromboembolic events; and estrogen causes additional adverse events.Trials of screening with fracture outcomes, screening intervals, and medications to reduce primary fractures, particularly those enrolling men, are lacking.Although methods to identify risk for osteoporotic fractures are available and medications to reduce fractures are effective, no trials directly evaluate screening effectiveness, harms, and intervals.Agency for Healthcare Research and Quality.

The American Geriatrics Society and British Geriatrics Society recommend that all adults older than 65 years be screened annually for a history of falls or balance impairment. The U.S. Preventive Services Task Force and American Academy of Family Physicians recommend exercise or physical therapy and vitamin D supplementation to prevent falls in community-dwelling older adults who are at increased risk of falls. Although the U.S. Preventive Services Task Force and American Academy of Family Physicians do not recommend routine multifactorial intervention to prevent falls in all community-dwelling older adults, they state that it may be appropriate in individual cases. The Centers for Disease Control and Prevention developed an algorithm to aid in the implementation of the American Geriatrics Society/British Geriatrics Society guideline. The algorithm suggests assessment and multifactorial intervention for those who have had two or more falls or one fall-related injury. Multifactorial interventions should include exercise, particularly balance, strength, and gait training; vitamin D supplementation with or without calcium; management of medications, especially psychoactive medications; home environment modification; and management of postural hypotension, vision problems, foot problems, and footwear. These interventions effectively decrease falls in the community, hospital, and nursing home settings. Fall prevention is reimbursed as part of the Medicare Annual Wellness Visit.
