Surveillance results of severe fever with thrombocytopenia syndrome in Anhui Province in 2024

Surveillance results of severe fever with thrombocytopenia syndrome in Anhui Province in 2024
Objective To understand the epidemiological characteristics and vector surveillance results of severe fever with thrombocytopenia syndrome (SFTS) in surveillance areas of Anhui Province in 2024, and to provide scientific reference for the prevention and treatment of SFTS. Methods The data of SFTS cases in 2024 from three national surveillance sites in Anhui Province (Chaohu City, Jinzhai County, and Hanshan County) were collected from the Chinese Disease Prevention and Control Information System. The epidemiological characteristics were analyzed by using descriptive epidemiological methods. Free-living ticks and parasitic ticks were monitored by using the cloth flag method and host tick detection method, respectively. Serum samples from healthy individuals at the monitoring sites were collected to detect IgG/IgM antibodies against the Bandavirus dabieense. Serum samples from domestic and wild animals were collected, and viral nucleic acid was detected by RT-PCR, and some samples were additionally tested for antibodies by using ELISA. Results In 2024, a total of 223 SFTS cases and 3 deaths were reported at three surveillance sites, with a mortality rate of 1.35%. The oneset time showed a bimodal distribution, with the main peak from April to July and the secondary peak from September to October. Among the cases, there were 94 males and 129 females, The age was mainly over 60 years old (79.37%, 177/223), and the occupation was mainly farmers (80.27%, 179/223). A total of 308 free-living ticks were captured, all of which were Haemaphysalis longicornis (longhorned ticks), with a density of 18.7 ticks/(hour·flag). A total of 194 animals were examined, and 1 041 parasitic ticks were captured. The proportion of Haemaphysalis longicornis was 99.04%, with a parasitic tick density of 5.37 ticks/host and a density index of 10.01 ticks/host. The tick infection rate was 53.61%. Among 1 000 serum samples from healthy individuals, the positive rates of IgG and IgM were 4.60% and 3.90%, respectively. The positive rate of antibodies detected by ELISA was 5.80% (4/69). The PT-PCR and ELISA tests for the wild animal specimens were both negative. Conclusion In 2024, the incidence of SFTS in the surveillance areas of Anhui Province showed a bimodal seasonal pattern, with elderly farmers being the main affected population. The density of free-living ticks and parasitic ticks in the environment is relatively high, with Haemaphysalis longicornis being the dominant tick species. There is a certain proportion of latent infections in healthy populations and domestic animals, and comprehensive prevention and control measures need to be further strengthened.

目的 了解安徽省部分地区硬蜱蜱种种类、蜱种分布及蜱虫携带无形体情况,为蜱传无形体防控提供参考依据。方法 于2023年4月至8月,在安徽省金寨县、含山县、旌德县、巢湖市等地通过布旗法和从动物身上采集硬蜱共计630只。用体式显微镜进行形态学鉴定,设计蜱特异性引物进行蜱种分子生物学鉴定。设计无形体16S rRNA特异性引物用巢式PCR检测单只蜱虫无形体携带情况。利用MEGA11.0构建系统发育树并分析。结果 安徽省金寨县微小扇头蜱占18.8%(18/96)、长角血蜱占81.2%(78/96),含山县、巢湖市均为长角血蜱,旌德县均为微小扇头蜱。长角血蜱携带无形体阳性率为30.9%(102/330),包括牛无形体1.8%(6/330)、嗜吞噬细胞无形体21.8%(72/330)和一种uncultured 无形体7.3%(24/330),微小扇头蜱携带嗜吞噬细胞无形体13.6%(18/132)。牛无形体和未培养无形体在游离蜱检测出,在24.4%的寄生蜱和15.8%的游离蜱检测出嗜吞噬细胞无形体。结论 安徽省调查县的蜱种包括长角血蜱和微小扇头蜱,长角血蜱和微小扇头蜱及其游离蜱和寄生蜱均携带无形体。

Severe fever with thrombocytopenia syndrome (SFTS) is a newly identified viral zoonosis caused by a phlebovirus. Most reported SFTS cases are farmers living in rural areas. The seroprevalence of SFTS virus in farmers has not been investigated. The current knowledge of SFTS virus seroprevalence in animals, especially in wild animals, is still poor.To investigate SFTS virus seroprevalence among farmers and a variety of animal species.SFTS virus antibodies in sera were determined using a double-antigen sandwich ELISA. Serum samples were collected from 2547 farmers and 2741 animals in 6 SFTS-endemic counties from March 2012 to February 2013 in Jiangsu province. The farmer participants aged from 15 to 90 years. All of them were interviewed using a structured questionnaire. The animals sampled included 6 domesticated animal species and 2 wild animal species.SFTSV antibodies were found in a total of 33 farmers (1.30%) and was more prevalent in males than in females (respectively 1.87% and 0.71%, P<0.01). The mean age of seropositive farmers was 56.5 years and seroprevalence increased gradually with age. Seroprevalence in animal species were: goats (66.8%), cattle (28.2%), dogs (7.4%), pigs (4.7%), chickens (1.2%), geese (1.7%), rodents (4.4%) and hedgehogs (2.7%). Multiple variable logistic regression analysis showed that grazing, grass mowing, raising cattle, age, farm work time and tick bites were risk factors for SFTS virus infection among farmers.SFTSV readily infects humans with farming-related exposures as well as numerous domestic and wild animals. Serological results further suggest that the virus circulates widely in Jiangsu province.Copyright © 2014 Elsevier B.V. All rights reserved.
