Pathological characteristics of opportunistic screening for upper gastrointestinal cancer and survival analysis of screen-positive patients in Ma'anshan from 2020 to 2023

Pathological characteristics of opportunistic screening for upper gastrointestinal cancer and survival analysis of screen-positive patients in Ma'anshan from 2020 to 2023
Objective To analyze the pathological detection results of opportunistic screening for upper gastrointestinal cancer (UGIC) in Ma'anshan from 2020 to 2023, and to compare the differences in cumulative survival rates among patients with positive lesions of different characteristics. Methods Pathological data from opportunistic screening for UGIC in Ma'anshan from 2020 to 2023 were obtained from the Information Management Platform for Early Cancer Diagnosis and Treatment Project (Rural Areas) to analyze the detection of lesions at different stages. Survival outcomes of patients with positive lesions were obtained by matching with the Ma'anshan Population Death Information Registration and Management System, with the follow-up deadline set as December 31, 2024. Results From 2020 to 2023, a total of 179 560 individuals underwent endoscopic screening for UGIC in Ma'anshan, among whom 76 527 patients received pathological biopsy. A total of 3 942 cases of precancerous lesions, 4 029 cases of early-stage cancer, 2 164 cases of advanced-stage cancer, and 6 193 cases of positive lesions were detected. The detection rates were 2.20% (3 942/179 560), 2.24% (4 029/179 560), 1.21% (2 164/179 560), and 3.45% (6 193/179 560), respectively, with an early diagnosis rate of 65.06% (4 029/6 193). The detection rate of total positive lesions was higher in males than in females (χ2=113.698, P<0.001), while the early diagnosis rate was lower in males than in females (χ2=579.083, P<0.001). The detection rate of total positive lesions was relatively high in the ≥70 years age group (7.63%), and the early diagnosis rate was highest in the <40 years age group (92.06%). Both the detection rate of total positive lesions (5.18%) and the early diagnosis rate (77.88%) were relatively high in the People's Hospital of Dangtu County. The 1, 2, 3, and 4 years old cumulative survival rates for patients with positive lesions were 80.09%, 77.61%, 75.93%, and 75.09%, respectively. The results of multivariate Cox regression analysis showed that age groups of 50-59 years, 60-69 years, and ≥70 years, as well as the presence of multiple lesions, were risk factors affecting patient survival time. Conclusion The detection rate and early diagnosis rate of upper gastrointestinal lesions in Ma'anshan City are higher than those in multiple regions. It is recommended to further strengthen health education for male populations and elderly groups to reduce the economic burden of upper gastrointestinal cancers in our city.

Description of the design and preliminary results of baseline recruitment and screening in the endoscopic screening for esophageal cancer in China (ESECC), the first randomised controlled trial (RCT) assessing efficacy and cost-effectiveness of endoscopic screening for esophageal squamous cell carcinoma (ESCC).ESECC trial is a cluster RCT, and 668 villages in rural Hua County, Henan Province, a high-incidence area of ESCC in China, were randomised into two arms at a ratio of 1:1. Screening arm participants were screened by Lugol chromoendoscopy; no screening was performed in the control arm. ESCC-specific and all-cause mortality, incidence of advanced ESCC and cost-effectiveness of screening will be evaluated in the next 10-year follow-up. Here, we report the performance of baseline recruitment and randomisation, prevalence of upper GI lesions and risk factors for ESCC.A total of 17 151 and 16 797 participants were enrolled in screening and control arms from January 2012 to September 2016. The truncated prevalence (aged 45-69 years) of oesophageal and overall upper GI high-grade lesions was 744.0/100 000 and 902.0/100 000. 69.9% of the 113 patients with high-grade oesophageal lesions were of early stage. Risk factors for severe oesophageal dysplasia and more severe lesions in this population included higher age, family history of ESCC, lower body mass index, eating rapidly and frequent ingestion of leftovers.This ESECC trial met the predesigned recruitment and randomisation requirements. Stomach and esophageal cancer exhibit high morbidity and mortality rate in China, resulting in substantial disease burdens. It is imperative to identify the temporal trends of stomach and esophageal cancer from 1990 to 2019 and project future trends until 2030, which can provide valuable information for planning effective management and prevention strategies.We collected and analysed data from the Global Burden of Disease (GBD) between 1990 and 2019, including incidence, mortality, disability-adjusted life years (DALYs), age-standardised incidence rate (ASIR), mortality rate (ASMR) and DALYs rate. We also calculated and reported the proportion of mortality and DALYs attributable to risk factors by sex in China and different regions. The Bayesian age-period-cohort model was applied to project future trends until 2030.The new cases, deaths and DALYs of stomach and esophageal cancer increased from 1990 to 2019. However, the ASIR, ASMR and age-standardised DALYs rates for stomach and esophageal cancer all decreased during the same period. These changes may be related to risks, such as smoking and diet. Furthermore, we utilised the projection model to estimate that the ASIR and ASMR of stomach and esophageal cancer among females will likely follow steady downward trends, while the ASMR of stomach cancer among males is expected to exhibit a significant decline. However, the ASIR of stomach and esophageal cancer and the ASMR of esophageal cancer among males are projected to display slight upward trends until 2030.The analysis of stomach and esophageal cancer trends in China from 1990 to 2030 reveals a general decline. However, it is crucial to acknowledge the persistent high burden of both cancers in the country. Adopting healthy lifestyle practices, including the reduction of tobacco and alcohol intake, avoidance of moldy foods and increased consumption of fresh fruits and vegetables can contribute to mitigating the risk of stomach and esophageal cancer. Significantly, the formulation and implementation of well-founded and efficacious public health policies are imperative for alleviating the disease burden in China.Copyright © 2024 by the Journal of Global Health. From 2003 to 2005, standardised 5-year cancer survival in China was much lower than in developed countries and varied substantially by geographical area. Monitoring population-level cancer survival is crucial to the understanding of the overall effectiveness of cancer care. We therefore aimed to investigate survival statistics for people with cancer in China between 2003 and 2015.We used population-based data from 17 cancer registries in China. Data for the study population was submitted by the end of July 31, 2016, with follow-up data on vital status obtained on Dec 31, 2015. We used anonymised, individual cancer registration records of patients (aged 0-99 years) diagnosed with primary, invasive cancers from 2003 to 2013. Patients eligible for inclusion had data for demographic characteristics, date of diagnosis, anatomical site, morphology, behaviour code, vital status, and last date of contact. We analysed 5-year relative survival by sex, age, and geographical area, for all cancers combined and 26 different cancer types, between 2003 and 2015. We stratified survival estimates by calendar period (2003-05, 2006-08, 2009-11, and 2012-15).There were 678 842 records of patients with invasive cancer who were diagnosed between 2003 and 2013. Of these records, 659 732 (97·2%) were eligible for inclusion in the final analyses. From 2003-05 to 2012-15, age-standardised 5-year relative survival increased substantially for all cancers combined, for both male and female patients, from 30·9% (95% CI 30·6-31·2) to 40·5% (40·3-40·7). Age-standardised 5-year relative survival also increased for most cancer types, including cancers of the uterus (average change per calendar period 5·5% [95% CI 2·5-8·5]), thyroid (5·4% [3·2-7·6]), cervix (4·5% [2·9-6·2]), and bone (3·2% [2·1-4·4]). In 2012-15, age-standardised 5-year survival for all patients with cancer was higher in urban areas (46·7%, 95% CI 46·5-47·0) than in rural areas (33·6%, 33·3-33·9), except for patients with oesophageal or cervical cancer; but improvements in survival were greater for patients residing in rural areas than in urban areas. Relative survival decreased with increasing age. The increasing trends in survival were consistent with the upward trends of medical expenditure of the country during the period studied.There was a marked overall increase in cancer survival from 2003 to 2015 in the population covered by these cancer registries in China, possibly reflecting advances in the quality of cancer care in these areas. The survival gap between urban and rural areas narrowed over time, although geographical differences in cancer survival remained.
